Reuse "As-Is" policy

In an attempt not to reinvent the wheel, does anyone have an "as-is" document they can share that end-users sign when receiving a reuse item? I am looking for something that the receiver would sign or initial that releases the organization of all liability and explains we have no responsibility if the equipment is faulty or needs repairs.
